County Health Rankings & Roadmaps Funding Award

Early in 2015 VtPHA received a small grant from the University of Wisconsin’s Population Health Institute which administers the County Health Rankings work under a grant from the Robert Wood Foundation. The grant was to “incorporate the contributors to health described in the County Health Rankings Model in the community assessments conducted by Vermont’s not-for-profit hospitals and their plans for community health improvement.” We hosted a series of conversations with all 12 Vermont Department of Health district directors, leaders at three Vermont hospitals, health department leadership, the Vermont Association of Hospitals and Health Systems and our own board. View the full report.
